Registration Cost Per Child: 13-15 year old $135.00

$25 late fee after February 27th.

Registration fee includes Shirt, Pants, Hat, and Socks.

NOTE: Please submit a copy of your players Birth Certificate with your registration, or bring it to your coach the first day of practice. If we already have your players Birth Certificate on file, please disregard this notice.

PARENTAL AGREEMENT Please select YES to indicate that you have read, and agree to each statement.
	I understand that each coach has been given a copy of the general provision & league rules of the BRL. I further understand that a copy will be made available to me by my commissioner upon request and can be found on the BRC website.
	I agree that my child may play in the BRC League & that their playing is of their own free will. I further agree that I will hold harmless the BRC League, BRC or its associates including but not limited to board members, commissioners, coaches & volunteers for any injuries sustained by my child in connection with any game or practice sponsored by the BRC League.
	I understand that travel to and from games and practices is my responsibility. In case of inclement weather, I must use my own judgment whether or not to attend a game or practice. Rain out information will be posted at www.bethesdarec.org.
	As a parent or legal guardian of the player named above, I hereby give my consent for emergency medical and/or treatment in the event of any injury associated with a baseball game or practice sponsored by the BRC League. This care may be given under whatever conditions are necessary to preserve the life, limb, or well being of my dependant.
	I understand that as part of my registration fee, insurance had been provided for my player. It is my responsibility to inform my coach and commissioner when or if my player is injured during a practice or game.
	I agree to allow any photographs or video footage taken of my child while participating in the BRL baseball season to be displayed on the BRC website and/or other media outlets as seen appropriate by the BRC board of directors or its members.
	By registering my child for baseball through the BRC League, I am a standing member of BRC and entitled to all benefits of membership. I understand that the Board meets the first Tuesday of every month at 6:30 in the cafeteria unless that Tuesday falls on a holiday, in which the meeting is automatically moved to the following Tuesday. I am welcome at all meetings and encouraged to attend and participate.
	I should notify my commissioner if I have any questions. If my questions are not satisfied, I can contact the president, Chris Woodard at kcwdrd@charter.net. All contact information for board members is located on the BRC website.
	I understand that my child will receive his/her uniform as part of the registration fee. It is my responsibility to notify my commissioner if there is an error with my uniform within 2 days of receipt. Uniform errors that are the fault of the vendor will be corrected at no cost to participants. Miss-orders by participants will be charged to the player.
	I am aware that $35 of my registration fee is non-refundable and there are no refunds after the 1^st practice.
	As a parent of a participating BRC player, I will be expected to work the concession stand during the time(s) assigned to my child’s team or my team as a whole may take advantage of the buy-out option. Not working my assigned time or not participating in the buy-out will have an adverse effect on the outcome of my child’s game.
